The following questions can be added to an existing questionnaire, or used to create a new questionnaire, to help protect you and your patients for catching Covid-19.
Copy and paste the following questions into your chosen questionnaire on the Questionnaire builder on the Control Centre.
- Do you suspect that you may currently have Covid-19?
- Have you been in contact with someone suspected or confirmed of having Covid-19?
- Are you suffering from any of the symptoms of Covid-19 such as a fever, a cough or have you had a high temperature in the last 14 days?
- Do you have any underlying health concerns such as diabetes, cardiovascular disease, lung disease or immunodeficiency?
