A Spreadsheet containing example questionnaires.
They are:
- Interests – aimed at understanding the needs and desires of your patients
- Medical History – aimed at understanding the past medical history of your patient, and any conditions that might effect furutre treatments
- Photography Consent – aimed at getting consent for taking and using patient photos for both medical and marketing purposes
You will have to create the questionnaire using the Control Centre, and create each question individually – although you can copy and paste the questions from the spreadsheet below to save a significant amount of time. This is also the perfect time to adapt them to your specific needs.
